Care Coordinator (Senior Services): Responsible for developing a Plan of Care and coordinating services and supports for seniors and adults experiencing physical disabilities, and ensure that activities and services are provided appropriately for participants. Bachelors’ Degree in Nursing, Gerontology, Social Work or related field preferred, with background in long-term care, delivery of community-based services and staff supervision. Experience and/or specialized education may substitute for degree requirements on a year for year basis. Two years’ experience in working with the general public one-on-one and in groups, direct contact with families and/or the elderly, and one year full-time paid work experience with human services recipients and providers. Chore Provider On Call (Senior Services): Assists a client in maintaining their independence while allowing them to remain in their home. Tasks may include assisting with meal preparation, laundry, lawn upkeep. Responsibilities include assuring that health, safety, and recreational needs are observed, maintaining records and reports on services provided. Home/Community Provider, FT, PT and On-Call (Family Services): Implements a prescribed habilitation plan for eligible consumers while providing routine direct care in either the home or community. Works closely with family or guardian and agency to design, implement and modify strategies for daily routines. High school degree or equivalent. Must demonstrate proficiency in direct care activities and possess excellent communication, observational and organizational skills. Residential Site Supervisor, Exempt (Residential Services): Responsible for supervising, training and providing technical guidance to assigned Supported Living Specialists and consumers. The Residential Site Supervisor provides direct support for clients in addition to providing coaching for the Supported Living Specialist. Other duties include working closely with the Residential Team Leader in managing the day-to-day requirements, providing quality supports in a residential setting, covering Supported Living Specialists on scheduled absences, and identifying staff during emergency fill situations.
